I find your response a load of lazybones!! Palmerston north hospital answered this seems odd you would rather put this in the "to hard basket" we as public are aloud to know this its not the ombudsmans job !!

You should probably consider following up with a request as to why this information is not collated in any centralised fashion and how the DHB keep track of the overall levels of misdiagnosis to ensure that they're acceptable, or whether they merely evaluate on a case-by-case basis. Even if they can't provide you with the actual level, they still have to answer any further queries you have about their record keeping. If they have a reasonable explanation for why they don't keep records that way, great. If not, well, that gives you something to bring to the attention of the relevant MPs and/or the media.
