Please provide:
1. Details of the incident(s) that led to the ban, including any investigation conducted.

Good afternoon Mark.

 

Please find our response re your OIA query below.

 

1. Details of the incident(s) that led to the ban, including any
investigation conducted.

 

Please be advised your request is declined under Section 6(c) Maintenance
of law, and Section 9(2)(a).

This is because the incident in question is still under investigation and
to release any further details at this stage could prejudice the
investigation and any potential WorkSafe action, and/or breach our
employees privacy.

 

What we can advise is what is public, that the “ban” on working from
heights was put in place following a worker falling 11 meters from a mast
in October 2023.

 

2. Details of the ban, including scope, duration and conditions for
cessation of the ban

 

We can advise that since the fall from height incident in October 2023,
Kiwirail put a moratorium on working from heights until we developed a new
Critical Risk Management Standard for Fall from Height and undertaken a
review of all work at height tasks and current access controls to assess
whether current controls were being effectively applied or if improvements
were required.

 

This Standard has been approved by our Executive and Business Unit leaders
are now working through implementation of the new Risk Management
Standard.

 

In recognition of this, the work at height ‘ban’ has now been lifted, so
long as the task and controls have been reviewed and confirmed as
compliant with the new Standards.
