Dear Fire and Emergency New Zealand,

In April 2023 I requested a copy of the operational report for incident F3406059 which occurred at Evergreen Crescent, on 22 January 2022. The G&A Select Committee had been advised that this was one of two rural Op Reports completed in 2022.

Contrary to the advice to the G&A Committee, Fire and Emergency’s advised that an Op Report had been "prepared" but not "completed" and that I was welcome to make another request for this information at a future date.

Accordingly, I submit a further request for a copy of this report of an incident that occurred 18 months ago.

While the operational review of the fire at Evergreen Crescent has been completed, the report has not been finalised. Unfortunately there have been significant weather events and subsequent reviews which have resulted in the team having to reprioritise work. This includes finalising the report into the Evergreen Review. Accordingly we must refuse your request at this time as the report does not exist (section 18(e) of the OIA). However, draft recommendations were provided to the District Manager prior to the last fire season so he could address any issues he had. If you want to make a request for the draft recommendations, we can assess that under the OIA.

We note that in your request you refer to advice that the report had been prepared but not completed. In our previous correspondence to you, we clarified that the operations review was undertaken, but the final report was not yet complete.

Dear Official Information Requests,

Thank you for your prompt reply to my request. I will prefer to wait until the report is released so as to be able to see the recommendations in the context of the ana;ysis of the incident response.

With respect to the difference between "Undertaken" and "Completed" I refer to the question posed by the Select Committee as set out below. I believe that the intention of the question is quite clear and reasonably assumes that it would be understood that it in this instance "Undertaken" meant that the review had been conducted and a report released.

The above Operational reviews were carried out by Fire and Emergency’s operational assurance
team were funded within baseline. Fire and Emergency do not break down the costs associated
with each individual review.
