Dear OIA Requests / Chris James,

So I am a little confused - your letter states and I quote "The Ministry foes not investigate deaths caused by an approved medicine"

I have written correspondence from the coroner that states deaths caused by the Pfizer vaccine "Are being investigated by Medsafe".

Please provide the process documents from start to finish that Medsafe go through when investigating the deaths caused by the Pfizer vaccine?
Why in this instance are the deaths not investigated by the Coroner but Medsafe given there are so many unexplained deaths and we know the vaccine has caused at least three deaths?
Of the 48 cases where Medsafe stated there was insufficient information to assess the deaths please provide a sample of 5 reports (personal information redacted) that came to the conclusion that there was not enough information?
Why were these 48 cases not referred to the Coroner for an autopsy to determine the cause of death?

And given my confusion can you please confirm if the following statements are correct yes or no

- When someone dies from an approved medicine this is referred to the Coroner - Yes or No
- When someone dies unexpectedly from the Pfizer vaccine this is investigated by Medsafe and not referred to the Coroner - so Medsafe who approve the vaccine then investigate the deaths from the vaccine - Yes or No

So straight away of the bat they are looking to steer professionals away from saying it is the vaccine. It is now all starting to make sense.

Well anyone that has been following this new from day one what is going on. It is just fishing it out to find out who is responsible for cutting corners - Politicians, Director of Health, Medsafe and / or all of the above

Thank you for your follow up email. Each part of your request is responded
to below. 

 

And given my confusion can you please confirm if the following statements

investigated by Medsafe and not referred to the Coroner - so Medsafe who
approve the vaccine then investigate the deaths from the vaccine - Yes or
No 

   

Neither of these statements can be answered with a dichotomous answer.  As
you have been advised on many occasions in the past, the Act does not
support requests where statements are to put an agency for response,
couched as a request for official information. Information on the role of
the coroner is available at:

Police always inform a coroner when someone dies unexpectedly, violently
or in suspicious circumstances. If a doctor is unsure what caused a
person’s death, they will report it to the coroner. Should the coroner
decide to take jurisdiction for the case, it is their role to  find out
when, where, how and why the death happened. They will also work out
whether anything can be done differently so that similar deaths can be
prevented. The coroner’s role is outlined in section 4 of the Coroners Act
2006 at:

The Ministry, including Medsafe, is not involved in the coronial
investigation. Coroners are independent judicial officers working in the
Coroners Court, with administrative support provided by the Ministry of
Justice. 

Please provide the process documents from start to finish that Medsafe go
through when investigating the deaths caused by the Pfizer vaccine? 

 

Why in this instance are the deaths not investigated by the Coroner but

As you have been advised, Medsafe does not investigate deaths. If a report
of someone dying is made to the Centre for Adverse Reactions Monitoring
(CARM) at the University of Otago, they investigate whether the vaccine
might have caused any adverse reaction in that person and note whether the
reported reaction has the potential to cause death. The actual cause of
death is determined either by the doctor or the coroner if the doctor
refers the case. 

Of the 48 cases where Medsafe stated there was insufficient information to
assess the deaths please provide a sample of 5 reports (personal
information redacted) that came to the conclusion that there was not
enough information? 

 

This is withheld under section 9(2)(a), to protect the privacy of natural
persons, including deceased natural persons. It has been confirmed by the
Ombudsman that copies of these reports should not be released. For more

Why were these 48 cases not referred to the Coroner for an autopsy to
determine the cause of death? 

 

Coroners decide if a post-mortem or autopsy should be done to find out
what caused a person’s death. As the courts are not subject to the Act
under section 2(6)(a), this part of your request is refused under section
18(g) of the Act on the grounds that information is not held by the
Ministry and there are no reasons to believe it is held by an agency
subject to the Act.  Further information about process for following up
reports of significant adverse events with COVID-19 vaccines is available

Under section 28(3) of the Act, you have the right to ask the Ombudsman to
review any decisions made under this request. The Ombudsman may be
