National Security Check for Permanent Resident Visa

There are news that Immigration New Zealand recently requesting some Permanent Resident Visa (PRV) applicants to complete the National Security Check form (INZ 1209).

As my knowledge, the INZ 1209 form has not normally been a requirement for the PRV applicants since the implementation of the Immigration Act 2009. However, it seems that it has been compulsory for applicants of PRV if they are from certain countries.

I would to request the following information:
1. When has INZ1209 become a compulsory for applicants of PRV if they are from certain countries?
2. If it is the compulsory reqruirement, why there is no relevant information for this requirement in any government sources, such as the INZ website.
3.Should PRV applicants provide the INZ1209 form with their PRV application forms, if it is compulsory?
4. Who did make this decision to require PRV applicants to provide INZ1209 form?
5. Which section of the immigration operational manual does outline this requirement?
6. The reasons for changing INZ1209 form was generally not required for PRV applicants to INZ 1209 form bcomes compulsory for PRV applicants from certain countries.

• NZ citizen or permanent resident - a copy of your passport (non New Zealand passport holders may include a copy of their residence visa); or
• In New Zealand - evidence of your New Zealand residential address (e.g. rental agreements, addressed mail, etc.) and a copy of your passport.
