Dear Tracey Martin,
With great respect, I request that your office would feel able to provide personal information of myself, Mr Ofa Haunga (dob 4th March 1967) under OIA so that I can be able to update my situation at this stage.
I was an employee of Oranga Tamariki and I had an issue with work colleague at workplace of which to this day I very much wish to be informed of any important information your office might have kept under my name.
My current residential address is, 24 Quinns Road, Shirley, Christchurch(8013), New Zealand. My Home telephone number is (03) 386 0692 and my mobile number is 027 4710336.
