In regards to details of a private individuals compassionate application being made public, I request information under the official information act

Under what policy was this information passed to the minister of health, Chris Hipkins, any other agency or ministerial office or staff from the ministry of health or the all of government response to covid 19 response team?

Who provided details of representations made on behalf of the pair to MP Chris Hipkins? Title and agency is sufficient.

Are you going to undertake an investigation into this privacy breach? As the question asked by MP Chris Hipkins was asked in his role as a member of parliament, not as a Minister. Private information about an individuals compassionate application has been passed on to member's of parliament who do not have ministerial responsibility for the information.

How have your actions complied with the privacy act 1993, especially principle 10(1)(b)?

How have these actions aligned with the governments ethos to 'be kind'?'

to respond to your request under section 15A of the Official Information
Act 1982 (the Act) as further consultation is required.
