Job Interview Questions - File and Notes Taken

S. Paurini made this Official Information request to Capital and Coast District Health Board

The request was partially successful.

From: S. Paurini

Dear Capital and Coast District Health Board

Job interview: part-time role as a Ward Administrator (ref: 104233748) at Wellington Hospital.

1. Please provide the full file including pre-interview and post interview emails, paper notes and any reports. Also provide the question forms used, the notes taken by the interviewers -Melanie Allen and George Shaw - in response to my answers.

2. Please provide a copy of your employment policy for: people with disabilities, Maori and older applicants.

Since I first applied, I have noticed the same role has been advertised again, twice. I have questions about my further application (plus previous applications) but I will wait until I read through the file and notes made regarding the interview with Melanie Allen and George Shaw.

Yours faithfully,

S. Paurini

Good morning S Paurini

I do apologise: three attachments were supposed to be attached to the email that I sent you at 14:07 on Thursday 13 February 2020.

I have now attached them to this email, with a second copy of the same response letter we sent you yesterday.

That was my administrative mistake, rather than that of the people who compiled the response.

I apologise for the inconvenience my mistake caused you.

Thank you, and please feel free to get in touch with me should there be any further issues.

Does your below further request for 'the full file - electronic and hard copy notes - of all information (other than [your] medical records) the CCDHB holds on [you]' still stand?

Kind regards

Catherine Stewart

Good morning S. Paurini

Thank you for your email below, and apologies that you have not yet received a formal acknowledgment from Capital & Coast District Health Board (CCDHB).

Upon reading your email, I followed up with the relevant team, and they have recently informed me that they will formally acknowledge your request as soon as they are able. Short of a formal acknowledgement, I can confirm that your request has been sighted by the relevant team and is being worked on.

The reason I cannot deal with this particular request is because I do not have access to your information, and am limited to handling Official Information Act requests. Because your request specifically pertains to all information that CCDHB holds on you, it is a Privacy Act request, and is beyond my current remit.

If there are any further issues, please feel free to get in touch, and I will pass your concerns on to the relevant staff member.

Thank you again
