Dear Capital and Coast District Health Board,
I write to you in my role as the Advocate of the late Samuel Jacob Fischer who was a patient of Te Whare o Matairangi psychiatric inpatient facility (TWOM) between January 31st 2015 to the time of his discharge to the Wellington Hosptial's Intensive Care Unit where he died on the 20th April 2015.
Based on the High Court recognition that “it is fundamental to the Official Information Act that the public are to be given worthwhile information about how the public’s money and affairs are being used and conducted, subject only to the statutory restraints and exceptions” I request the following information under the provisions of the Official Information Act 1982:
1) Any credit card statements, expense reimbursement claims, invoices or receipts from the Medical Director at Mental Health, Addiction & Intellectual Disability Service 3DHB, which are in relation to the Medical Directors attendance of the Coronary Inquest into the death of Samuel Jacob Fischer on 23rd, 24th, 25th, 26th and 31st July.
2) Any personal expenses incurred during the period noted in point 1, either on business or personal credit cards, and names of individuals that may appear in the expense information.
3) All transaction dates, names, and total amounts and purpose of the transaction and the names of the parties to the transaction.
I understand that a decision on a request for information under the Offical Informations Act 1982 should be made within 20 working days of receiving that request.
