Dear Capital and Coast District Health Board,

I write to you as the administrator of my sons estate, the late Samuel Jacob Fischer who was a patient at Te Whare o Matairangi psychiatric inpatient facility between January 31st 2015 to the time of his discharge to the Wellington Hosptial's Intensive Care Unit where he died on the 20th April 2015.

I request, pursuant to any legislation including but not withstanding the Official Information Act 1982, and/or the Coroners Act 2006, and/or the Health Act 1956, and/or the Health Information Privacy Code, the following evidential reports:

1) Report Type: Activity Report

My Expert Advisor Mr Jak Wild has informed me that he has in his possessions examples of such Activity Reports which are included in a 'Coronial Inquest Documentation Bundle' provided to a family Mr Wild is also advising, whose son also died whilst a patient in Te Whare o Matairangi.

For this reason my expectation is for a prompt and complete release of the information requested given the precedent Mr Wild has informed me of, due to the electronic ease in which information can be collated, and due to the urgency required for ongoing investigations into my sons death, including a part-heard Coronial Inquest.
