Dear Capital and Coast District Health Board,

I write to you as the administrator of my sons estate, the late Samuel Jacob Fischer who was a patient at Te Whare o Matairangi psychiatric inpatient facility between January 31st 2015 to the time of his discharge to the Wellington Hosptial's Intensive Care Unit where he died on the 20th April 2015.

I request, pursuant to any legislation including but not withstanding the Official Information Act 1982, and/or the Coroners Act 2006, and/or the Health Act 1956, and/or the Health Information Privacy Code, the following evidential documentation:

Reporting Period: 0700am to 1800pm, 17.04.15

My Expert Advisor Mr Jak Wild has informed me that he has been provided One Hour Observation Forms for other patients he has Advocated for at Te Whare o Matairangi. This includes on one occasion when these forms were released as part of a standard patient health information request, and on another occasion when these forms has been released to a family he is Advising who have the One Hour Observation Forms included in their documentation release as part of their Coronial Inquest Bundle.

For this reason my expectation is for a prompt release of the information requested, given the precedent Mr Wild has informed me of, due to the limited amount of information and ease in which information can be collated, and that this request is under urgency as it is information I require for ongoing investigations into my sons death, including a part-heard Coronial Inquest.

My preference is for the information to be released in electronic form. If it is in paper form please have the information available for collection from the Wellington Hospital reception desk.
