Please provide the following information regarding the reported banning of Martyn Bradbury from Radio New Zealand National after his comments on Jim Mora's The Panel (Thursday 6th October 2011):
1. Who made the decision to ban Martyn Bradbury?
2. Did this decision follow any communication with other parties (within the Radio NZ organisation and/or with outside bodies or individuals)?
3. Please provide any internal & external correspondence (emails, letters, etc) leading up to this decision and also any correspondence relevant to question 2 above.
