Dear Ministry of Social Development,

Dear Accident Compensation Corporation,

Much has been made of the accidental release of personal information by government agencies.

I'm interested in situations where government agencies have intended to release personal information.

For example to a private investigator, a credit agency etc.

My questions, with regard to the above, are

(a) Has your agency every released personal information to any agency like a private investigator, credit agency, etc ?

(b) If information has been released, who has the information been released to ?

(c) What personal information has been disclosed ?

(d) On how many occasions has this occurred ?
