As this information is readily available to ACC Government Services, and required to first seek the internal review of 2 treatment injury claims lodged through White Cross Medical Center in Ponsonby on 15/08/17, prior to lodging a formal review through ACC's review application process, I would appreciate urgency being applied in this instance respectfully requesting that the Corporation provide the requested information within the next 5 working days from today.

Thank you for your response.
I advise I have checked my emails and I have no records of receiving by way of email, and or letter from the Corporation's Ms Emellia McLaren, and certainly not an email received from the [email address] on 27/11/17, any email correspondence in line with the information that was provided to me by Government Services on 4/3/19.
As you will note over 15 1/2 months have passed since Ms Emellia Mclaren's alledged correspondence was dated, and for which I saw for the first time on 4/3/19.
During this period Ms Emellia McLaren may have been promoted elsewhere within the Corporation, or have left.
I am also aware that ACC have applied the term manager to groups of people that manage other staff within the Corporation, but may not be the actual senior manager of a unit. So I remain unsure if Ms Emellia McLaren's 27/11/17 designation of Team Manager, Claims Assessment & Advice Treatment Injury Cover Assessment Centre definitively means that Ms McLaren is in fact the Senior Manager of this unit, or whether she remains in this role currently.
Ms Emellia McLaren's ACC email address is also not shown within the email sent by Government Services, nor any of the correspondence attached to said email.
My records indicate that Ms McLaren, and or her team have erred in the processing of my 2 treatment injury claims submitted by the Doctor to ACC on 15/08/17 in writing as per my correspondence dated 15/8/17. The Courts have found that a claim need not have to be submitted on the Corporation's system's and or claim forms to be valid. The GP on 15/08/17 as confirmed in Ms Emellia MacLaren's email sent my correspondence to ACC on the 15/08/17 by way of email and thus my lodgments of the two injury claims remains valid.
Furthermore the investigations that ACC carried out as Ms Emellia McLaren referred to in her dated correspondence of 27/11/17, which I have never seen before 4/3/19, acknowledges that the Corporation had a duty to investigate these 2 claims. An acceptance would allow me to seek the appropriate entitlements in terms of further assessment and treatment that I have requested in my letters of 15/08/17. ACC's declinature would allow me to seek recourse through the Corporations 2 separate decline letters which I could either take to review or seek a decision reviewed by the district courts.
The two treatment injury claims pertain to;
(1) Claim No: 10018973176,
(2) and a new accidental injury claim specified in my letters of 15/08/17, assessed, diagnosed, treated and recorded by my then principal treatment provider who was a physiotherapist and who billed and received payment for treatment of this second accidental injury but failed to lodge a claim with the Corporation for the injury. This failure has lead to the delay in me having cover for these new injuries. All future treatment providers, acc staff, assessors, who have access to the Corporation's classes of injuries covered under each accident, therefore have no record of me sustaining a second and co-morbid injuries, and therefore they have never been further assessed, treated and rehabbed and remain a barrier for me to return to building.
Instead of correctly processing the 2 new treatment injury claims as separate and new treatment injury claims, staff at the Treatment Injury Center have incorrectly processed the treatment injury claim under a prior and totally unrelated treatment injury claim for injury that was sustained after deep accupunturing by a physiotherapist, and processed under acc claim no: 10031989043.
This has meant that the Corporation's incorrect actions have lead to me not recieving 2 valid decision letters on the treatment injury claims 1 under claim number 10018973176, and the second one for the claim when I was lying down and raised and turned on my bed to pick up my young son then turned back with him as I twisted and lay back again and when I prolapsed my back at L5/S1 as is clearly stated in medical notes supplied to ACC in March April 2012 by New Lynn Physiotherapy, West Auckland Physiotherapy, and corroborated by my written account of events supplied to Stephannie Bennebroek and Leanne Ashby and held on ACC's file.
It is therefore with interest I seek confirmation as to whether within the Treatment Injury Centre Ms Emellia McLaren,
OIA Request 1
(1) is the most senior manager in the Treatment Injury Centre, or whether within the treatment Injury Centre Ms Emellia McLaren has a more senior manager that she is responsible to. If this is the case it would be appreciated if you could advise me of the most senior manager within the Treatment Injury Claims Center, and their email address please.
OIA request 2
(2) If Ms Emellia McLaren, is currently the most senior manager within the treatment injury centre, please confirm to me by return response Ms Emellia McLaren's email address so that I can correspond with her in relation to the above matters and seek redress for the processing errors that ACC have introduced which I have highlighted above and which unreasonably deny me of my rights to be able to appeal the decliniture of the 2 treatment injury claims submitted to ACC by GP on 15/08/17.

(3) Please provide confirmation of the name and email address to which Ms Emellia McLaren is answerable to within ACC if this information has not already been provided in your answer to (1) above.
(4) It would be helpful if you could advise me which acc claim number the Treatment Injury Claim's Center and Ms Emellia McLaren have saved all the correspondence to ACC in relation to this matter, inclusive of Ms Emellia McLaren's purported email of 27/11/17 to which I have referred to in this correspondence.
ACC's prior advice has been for claimants to first seek redress with the management of the unit for which errors or corrections surrounding pending covers arise. I am therefore trying to establish who internally at ACC, I raise the above concerns with when the mistakes, and seek redress/reveiw on matters that I have highlighted occurred under Ms Emellia McLaren's oversight.
I respectfully request that urgency be applied to my request for ACC's assistance in these matters and your response is returned by the end of business close on Wednesday 13/0319 since this information is already known to ACC Government Services.
Of course ACC's treatment Injury Centre's senior manager, could always contact me by email which is known to all parties using their acc email address in a conciliatory effort to commence engagement in order for these matters to be resolved . At this point we could enter into some meaningful dialogue in order to resolve the matters that I have raised today, and reflect what I have stated on 8/11/17.
Otherwise my first option is to correspond by email with the Senior Manager of ACC's Treatment Injury Centre, or Ms Emellia McLaren's senior manager if Ms McLaren currently holds such role, and I am therefore totally reliant on receiving back from you this official information that I have requested to further this process.

I am more than happy for Government Services to provide the full Official Information requested above to my personal email address, if there is a concern about privacy of any staff's email addresses being displayed on the FYI website. In fact I would expect that this would be the case.
I note that to date Government Services has not returned the requested Official Information of 11/3/19 to my personal email address , which will aide me being able to communicate by email with the Senior Manager of the Treatment Injury Claims Unit.
Once ACC Government Services provides me with the Official Information requested above, I will be able to initiate remedial communication with the Senior Manager of the Treatment Injury Claims Unit, or Ms Emellia McLaren's Manager, if Ms Emellia McLaren is the Senior TIC Manager.
Written confirmation back of the positions and email addresses requested in my official information requests of 11/3/19, will provide accurate and current contact names and email address details for me to be able to initiate remedial processes by email which will allow me to retain an email trail of our pending communications. It will also act as an ACC formal response which the Corporation can not dispute should at a later stage I need to engage the assistance of the Office of the Ombudsman.
I am of the opinion that it is unreasonable for the Corporation to provide me details of the official information contact names and email addresses requested by way of a telephone call which at a later stage could be easily disputed, should my correspondence go unanswered. A written response to my personal email address with the Official Information I have requested in full, will leave no room for human error and later ambiguity of the correctness of the official information provided to me.
I note that my request for this official information from ACC Government Services was applied for under urgency by the end of business close on Wednesday 13/3/19, which is tomorrow, and so I will look forward to receiving back under urgency and in full the Official Information that I requested on 11/03/19 which I have copied agin above for your quick reference. The positions, names and contact email addresses that I have requested under the OIA, is easily locatable by Government Services, and any delay in ACC's response past 4.30 p.m. on 11/3/19, I suggest only acts to further unreasonably delay the necessary assessment, treatment and rehabilitation for the injuries that have never been investigated.
