In ACC's Executive Leadership Team Issues Paper dated 16 February 2009 subject being the "Strategy for the Future Management of Long-term Claims" it reads under paragraph 5.5 on page 4 as follows which I quote;

"5.5 While the Service Delivery Model has provided a framework for the management of 'high complexity' claims, there is currently no strategy for the management of LTWC."

OIA Request 1;

I respectfully request the provision of a full copy of the ACC "Service Delivery Model has provided a framework for the management of 'high complexity' claims" that was in force as at the 16 February 2009, and

OIA Request 2;

Please also provide me with copies of all subsequent iterations of ACC's "Service Delivery Model has provided a framework for the management of 'high complexity' claims" that have been updated through to present.

Thank your for correspondence dated 28 June 2018 forwarded to my personal email address and received at 1.00 pm on the same date.

A copy of your letter to which I respond, is quoted in full following my sign off below for which I note that you have grouped three of my different Official Information requests including this request. In order to keep things in order for myself, should I be required to seek the assistance of the Office of the Ombudsman, I have responded to separately to your request of 28 June 2018, through www.FYI.org.nz, specifically to the original requests that I have made.

I note that my request for a full copy of the ACC "Service Delivery Model has provided a framework for the management of 'high complexity' claims" that was in force as at the 16 February 2009, and copies of all further iterations through to present is a specific and narrow request for Official information in relation to one of ACC's case management streams as at 2009.

The provision of a copy of ACC's Service Delivery Model that provides a framework for the management of 'high complexity' claims will provide me with, among other official information,

- the basis for what ACC deem to be "high complexity" claims,
- provide me with an understanding of the approach that ACC applied at 2009, and covering the period of my claims with ACC through until present, to the case management of more complex claims, in contrast to how my claims have been managed through 2012 to present, given that ACC's contracted assessor Mr Vasu Pai, declined to undertake the medical case assessment of my injuries as requested by ACC, advising my then case manager Ms Leanne McDonnell by email on the 19-12-2014 at 8.30 (a copy of which is located on my claim file 10018973176 and for which I have sent you a separate copy by email for quick reference) the following and I quote;

"I would be grateful if you could cancel this appointment from my list on 23 December as it is far too complex...."
- ampong other injuries, I have accidental injuries that relate to the cranial/facial nerves, cervical, thoracic and lumbar spine, shoulder, elbow and hands, which I am contiuing to seek ACC's support for the proper assessment, treatment and reabilitation.

- the information once received will be reviewed, and potentially be used for review and appeal purposes in relation to matters involving several claims, so that I can get the necessary entitlements to treat these unhealed injuries.

I trust that my expressed interest satisfies your imposed interest test, and I look forward to receiving the full information as requested on the 13 June 2018, pursuant to the time provisions of the OIA from 12 June 2018.
