Dear Accident Compensation Corporation,

I respectfully request that ACC provide me under the Official Information Act the full list of ACC Code Descriptions that ACC apply as part of the Administrative Review when an ACC client applies for a review.

To avoid any confusion I have listed several Code Descriptions that will be included in the full list of Code descriptions that I am seeking and which are coded under the Administrative review section of a claim file when a claimant has applied for review;

Code description: X8 Suspension/Cessation of Entitlements
Code description: Z1 Physical Treatment Regulations
Code description: Z5 Jurisdiction - Other

I respectfully welcome the full list of ACC Code Descriptions that ACC use when completing the Code Description field on the Administrative review section of a clients claim file, being provided to me under urgency within 5 working days from today, being by the end of business close on 19th September 2017.

In line with section 12 (3) of the Official Information Act, I list my reasonable reasons for urgency below;

1. The information requested is readily held by ACC, and thus is easily retrievable for the supply to me with in the requested time frame,

2. The information requested is required to enable me to establish the validity of the Code descriptions that have been previously, and will be allocated in future by ACC staff when conducting Administrative reviews for the purposes of review of ACC's decisions and the alternative dispute resolution options that were available to myself.

3. This information is required to be used in current, alternative dispute resolution, review and appeal proceedings.

4. This information will allow me to work with ACC to ensure that prior to review proceedings commencing, in some of the more recent applications for review, that ACC and I agree completely that the Code descriptions that have been allocated are in fact the correct and legitimate codes that will validate the alternative dispute resolution process that follows.

5. Agreement of the correct Code description will ensure that my applications for reviews avoid any and all unreasonable delay in the alternative dispute resolution/review process, that would further unreasonably delay the Corporations actions that will prolong me being able to get claimed injuries properly assessed, treated and rehabilitated.

I confirm that under s16(2) of the Official Information Act, my preferred way of receiving a response is by email to the address from which ACC received the original request, not by post.
