Good afternoon Destiny,

My name is Rory I am the OIA Coordinator here at DHB Shared Services, assisting DHBs with in common OIA requests. I note your request copied below has gone to 12 DHBs.

The DHBs have noted the scope of this request is very large including a long timeframe (20 years). They also note that some of the information wouldn't be able to be provided. I have set out the information/questions they have provided below and ask that you consider narrowing the scope of your request. I am happy to assist with this further.

1. Is this request covering Mental Health only, or is it any service in the DHB where a person has presented with one or more of these symptoms/diagnoses? Should it include ED admissions?

2. If the request is Mental Health only, should the data be for inpatient admissions only, or is it for any Mental Health Service contact (i.e. community services)?

3. We can supply data on admissions and/or patients seen in Mental Health Service with the following requested diagnoses (or associated diagnostic groups):
- Diagnosis of bipolar disorder
- Anxiety
- Depression

We can't supply data on admissions and/or patients seen in Mental Health Service with the following requested symptoms:
- Seizures
- Insomnia
- Abnormal bleeding (What kind of abnormal bleeding? Self-harm related, other physiological causes?)

4. With regards to suicide information, as per above, what is the scope of the request (MH Services, ED data, inpatient only, numbers of admissions or number of patients)?:
- Suicidal thoughts -This is information generally held in clinical files, the vast majority of which is paper based going back 20 years, therefore there would be significant hours of work involved.
- Suicide attempts - This will also be difficult to report accurately as per the previous point. We could supply Safety 1st/RM Pro incidents where a reported suicide attempt has been captured, but this will likely not be complete, and would only cover around the last 5 years.
- Death by suicide - Again, what is the scope? All suicides in the District, Mental Health inpatients only?

If the request is unable to be narrowed the DHBs may have no option but to attach a charge for the significant work involved, or refuse the request on the grounds of substantial collation required under Section 18(f). But we are wanting to work with you to get something manageable so they can provide a response. If there is any information you can provide about what you are wanting to use this information for it would be helpful too. A narrowing of the timeframe for the information requested would help to make this manageable, as per the comments above, depending on what you are wanting the information for.
