I request the process in which ACC are notified of a death to a person who is receiving weekly compensation, ie by the deceased's doctor, forms completed and the codes used by the doctor.

What is the process ACC do in closing the currently active file when advised of the claimants death and the codes used.

how does a GP update the ACC45 and current ACC18 after the claimants
death.

 

Please confirm whether ACC has any form of contact after a claimants death
with the coroner of there officers.

 

Please provide the scope of information recorded about the death and now
specific  ie illness, accident, heart failure, car accident, work place,
self harm, firearms etc

 

What information produced in the monthly Accurate Client Information (ACI
) Date of death exception report.

 

Is the Date of death exception report only cover the prior month or what
period do it cover.

Therefore please confirm that ACC do not record the cause of a clients death whether accident or illness. simply answer "Yes" or "No"

As previously advised in our letter, the information ACC records, about the cause of a client's death, depends on individual circumstances relating to each claim.

In some situations we, would record the cause of death on the deceased client's file and in some situations we do not.

Our previous responses have provided the detail related to your questions.
