Dear Ministry for Primary Industries,

I would like copies of the Animal Health Certificate which was signed off by Dr Adrian Rodrigues of Ministry of Primary Industries for a dog that was flying on flight NZ02 departing Auckland to London on 27th July 2015 at 21:40.

Please include documentation used to check and verify the correct crates size had been used for the flight, including reference to guidelines and standards used.
