Under the OIA, I request all information, documentation, costings, advice sought & provided, correspondence between suppliers, manufacturers, pharmacy wholesalers, regarding the management of supply of Stelazine (trifluoperazine).
I also request all documentation & advise received/sought from patients, customers, MPs, pharmacists/pharmacies, doctors, nurses/all health care professionals regarding the supply of Stelazine (trifluoperazine) in recent times. The names/addresses can be blanked out.

Thank you for your request for official information, received by the
Ministry of Health on 4 January 2017. The Ministry's reference number for

You requested:
I request all information, documentation, costings, advice sought &
provided, correspondence between suppliers, manufacturers, pharmacy
wholesalers, regarding the management of supply of Stelazine
(trifluoperazine).
I also request all documentation & advise received/sought from patients,
customers, MPs, pharmacists/pharmacies, doctors, nurses/all health care
professionals regarding the supply of Stelazine (trifluoperazine) in
recent times. The names/addresses can be blanked out.
Your request has been received and logged. You can expect a reply no
later than 13 February 2017 as required under the Official Information Act
1982.
Please note that a working day is defined in the Act as ‘any day of the
week other than a Saturday, Sunday, Good Friday, Easter Monday, Anzac Day,
Labour Day, the Sovereigns birthday, Waitangi Day and a day in the period
commencing 25 December in any year and ending 15 January in the following
year.
If a large amount of information has been requested or if the Ministry
needs to consult in order to make a decision, we may need to extend this
date (this is provided for in section 15A of the Act). We will advise you
if such an extension is necessary.
With respect to this request and your two other requests, my apologies but
this is the first record I have of your request having been made. It's
standard practise to send an acknowledgement letter to the requester,
which includes a statement of the request as we understand it, an expected
due date, and a reference number for the particular request. In the
absence of that, we are treating this as a new request made by you as of
today's date. The due date I've given you is in accordance with the
non-working day period as laid out in the Act, and made using the
calculator that is located on the Ombudsman's website.
We have notified FYI.org that there appears to be an issue with requests
being submitted by some users from their site.
