Dear Ministry of Health,

I request information under the Official Information Act regarding Wayne Baker, Pharmacist, ALL advice sought and received, documentation, evidence, a breakdown of offences, and correspondence between Officials, and between Officials and WAYNE BAKER, Pharmacist, of Auckland who was convicted of falsifying scripts between 2006 and 2009.

I also request ALL documentation relating to the initial 'tip off' of Mr Baker's investigation, in particular who (a name is not required, but occupation), and when (i.e. what month and year). Also, how long did it take after this tip off occurred, and Mr Baker getting interviewed, and later charged of defrauding the MOH.

I also request any documentation, and advice provided or sought by the Pharmacy Council, and/or the Health Practitioners Disciplinary Tribunal regarding Wayne Baker's suspension, and readmission to the register of pharmacists, in relation to the above incident.

You requested:
I request information under the Official Information Act regarding Wayne

Your request has been received and logged.  You can expect a reply no
later than 26 August 2016 as required under the Official Information Act
1982.

Please note that a working day is defined in the Act as ‘any day of the
week other than a Saturday, Sunday, Good Friday, Easter Monday, Anzac Day,
Labour Day, the Sovereigns birthday, Waitangi Day and a day in the period
commencing 25 December in any year and ending 15 January in the following
year.

If a large amount of information has been requested or if the Ministry
needs to consult in order to make a decision, we may need to extend this
date (this is provided for in section 15A of the Act).  We will advise you
if such an extension is necessary.
