Serious and Sexual Assualts by Prison Staff

Dear Department of Corrections,

I would like to make a request under the Official Information Act.

Could you please provide me with all available data on the following:

1.The number of complaints over the past 5 years, made by prisoners incarcerated in New Zealand, of Serious Assault or Sexual Assault by Prison Staff.
a. Please provide a breakdown of this information by prison and gender of prisoner.
b. Please provide the number of these complaints which were investigated by the Department of Corrections or any other body or company responsible for the employment of Prison staff.
c. Please provide the number of these complaints which were upheld.
d. Please provide the results of the investigations.

2. Please provide the number of prison staff, currently employed in New Zealand, against whom a complaint of serious or sexual assault has been made.

Additionally, could you please provide me with the following documents:

1. Any policies relating to how a complaint by a prisoner against prison staff can be laid, including copies of any forms required to be filled out.

2. Any policies or procedures implemented by the Department of Corrections or any other body or company responsible for the employment of Prison staff for investigating these complaints.

3. Any policies or procedures to be implemented in the event of a complaint, to ensure the safety of an incarcerated complainant.

Following an investigation, the Ombudsman has upheld my complaint and the Department will be releasing the information to me and changing its Administrative procedures to ensure this information is collected in the future.

I am glad the Ombudsman was able to resolve this one - would you be willing to upload the response from the Ombudsman, or the reasoning they used? It would be helpful to know their reasoning to apply to other OIA requests. I would also love to see the data when you receive it
