Dear Counties Manukau District Health Board,

In response to concerns raised by a number of professional colleagues, I would like to please request any data the DHB possesses relating to pharmacist turnover rates (either by fiscal or calendar years) for the years 1996-2015.

In addition, if available, I would like to request the associated management structure of the CMDHB Pharmacy Department (or the equivalent department in previous administrations) at the end of each of the years 1996-2015.
