Kia ora,

For each of the past five financial years, please provide aggregated, de-identified information on the number of instances in which Wellington Water:

identified, or received notification of, newly installed works (including pipes, fittings, connections or property remediation) that:

-failed due to product defects,
-were installed incorrectly,
-did not meet required specifications or standards, or
-failed inspection or testing requirements.

For each financial year, please provide:

The total number of such instances
1. The number of instances where Wellington Water required the contractor to rectify, repair, or replace the work.
2. The number of instances where no rectification was requested by Wellington Water
3. The number of instances where the issue was identified:
-by contractor self-certification,
-by Council review or inspection,
-by Wellington Water inspection,
-by third-party inspection,
-by customer or property owner notification

Of the above, the number of instances where the work had previously been self-certified as compliant by the contractor

Where readily available, the number of instances involving repeat issues or multiple rectifications for the same installation

This request is limited to aggregated information and does not seek identification of individuals
