Section 25 Request Made to Insurance Companies for data re inter alia "churn"

Response to this request is long overdue. By law Financial Markets Authority should have responded by now (details and exceptions). The requester can complain to the Ombudsman.

Dear Financial Markets Authority,
I request to see a copy of the data request that you have made in 2015 to Insurance companies in regard. I do not want the answers from the companies but rather the template questionnaire that I assume you included with your request. If you did not include a template questionnaire, then I request whatever you asked a typical insurance company in the form that you asked them for data.
