Following my recent OIA Act request to Oranga Tamariki—Ministry for Children (see link here https://fyi.org.nz/request/32182-fale-an...) reference number: OIA CE 2025 03086 John FYI OIA response BH Signed.pdf, it has come to my attention that some of the information you published about Fale Andrew Lesā is incorrect. I am requesting that you amend the statement to reflect the accurate information.
Additionally, I would like to understand what steps were taken to verify the information before it was published. Specifically:
What fact-checking process was followed?
Was the information confirmed with the individual concerned prior to publication?
Did Fale Andrew Lesā consent to or approve the statement that was published?
I would appreciate your prompt attention to this matter and a clear explanation of your editorial process in this case.
