Dear Pharmaceutical Management Agency,

 

In respect of COVID-19 products (limited to those injectable medicines) to
enable identification of which entity :-

 

i) ordered the product

ii) imported the product; and

iii) the product was charged to

 

From March 2020 until July 2023 (inclusive) please provide for all imports

 

Copies of the

-purchase order form

-shipping form and/or waybill shipping

-Packing slip for product on arrival

 

All pricing information will be redacted.  Only the information required
to establish the above i)-iii) is requested.

 

This request has also been sent to MBIE and the Ministry of Health so the
three agencies can establish who is responsible for this request.  I trust
this approach will avoid the delays that can occur when requests are
passed between agencies.
