Please note, we have undertaken initial scoping of your request with our
subject matter experts who have advised that part of your request requires
refining. This is because starting masculinising or feminising hormone
therapy can be done by any medical professional who is authorised to
prescribe medicines. This means it will entail substantial collation due
to the significant number of consent forms across Health NZ services (such
as endocrinology, paediatrics services, child and adolescent mental health
services, etc). We propose an amendment of your request to a particular
region or service you are interested in. Providing clarity will help
reduce the scope and enable us to fulfill your request.
We appreciate you are under no obligation to amend your request, but
requests that require substantial collation may be refused under section
18(f) of the OIA. We want to work with you to avoid this and ensure we are
able to provide a response that best meets your needs.

Thank you for your response. Unfortunately, I failed to see the previous request for refinement of this OIA request.
I look forward to receiving Appendix 1, “a sample of consent forms we hold” which while referred to in your response was not attached to or part of your response to me.
In your response you state that:
“Each general practice and secondary services, just as hospitals may have their own version.”
Given the context, I am assuming you mean ‘their own version’ of consent forms for masculinising or feminising hormones?
Could you please clarify if this assumption is correct – i.e. the content of consent forms for either masculinising or feminising hormones may differ by each General Practice, Secondary Service or Hospital in New Zealand?
i.e there is no standardisation?

Please see the attached Appendix one. Apologies this was not attached when
sending through your OIA response.
In response to your below email, I can confirm that your assumption is
correct – each health care provider that offers masculinising or
feminising hormone therapy will have a consent form, and there could be
some variation in what is included in the consent form. Therefore, we
advised in our response that each provider will have their own version.
There is no overarching standardisation.
Consent forms are intended to document and ensure a patient’s
understanding as part of informed consent – which is a legally required
process. You can read about the requirements of obtaining informed consent
on the Medical Council’s website here: [1]Statement on Informed Consent.
Consent forms must therefore (at a minimum) document the requirements of
informed consent.
