Thank you for your email on 7 April 2025, asking Health New Zealand | Te Whatu Ora for the
following under the Of icial Information Act 1982 (the Act):
Please provide copies of the current consent forms used in NZ for:
a) starting masculinising hormone therapy
b) starting feminising hormone therapy
If these differ by region, please provide copies of the current consent forms for each
region.
Response
On 22 April 2025, Health NZ sought refinement/clarification to refine your request to a particular
region or service but to date, we have not received a response. Set out below is our decision on
your request.
We have provided a sample of consent forms we hold in
Appendix One and, are available on
HealthPathways for each hospital. HealthPathways provide a masculinising and a feminising
hormone therapy consent form, adapted from the Hauora Tāhine – Pathways to Transgender
Healthcare Services, for which hospitals have changed the letter header to display their local
district.
HealthPathways is an online tool that offers clinicians locally agreed information to make the right
decisions, together with patients, at the point of care. HealthPathways is written for general
practice teams but is available for use by all registered health professionals. It contains complex
clinical content and service access information for clinician use only so is not open to the public.
Each general practice and secondary services, just as hospitals may have their own version.
However, as your request is currently worded, your request is very broad and substantial manual
collation would be required to locate and prepare all documents within scope of your request. As
such, we are refusing part of your request under section 18(f) of the OIA.
