In accordance with the Official Information Act please provide the below information.
How many referrals for each year for the last 5 years has ACC made to Driver Solutions, an occupational therapy transport assessment service based in Dunedin.
For each year for the last 5 years how much money has ACC paid to Driver Solutions.
Please provide a copy of the contract that Driver Solutions has with ACC.

In your response the the OIA request you state that Driver Solutions is not a vendor to ACC, yet Driver Solutions website states they are an ACC vendor.
Please see the below link and scroll to the bottom of the page where it clearly states Driver Solutions are an "ACC vendor" and if clicking that "ACC vendor" link it takes you to ACC's website.
