I am writing to formally request a full copy of the police disclosure relating to my charge for trespassing on KiwiRail land on November 10, 2022. This charge was later withdrawn by the Police Prosecution Services. I request the following documents and information:

Full Police Disclosure:

All records regarding the charge of trespassing, including handwritten or digital notes from both arresting officers (including Officer Julia Hall of Wellington Police, whose name I already have).
A full copy of the arrest documents, including any forms or reports filled out during or after the arrest.
A record of who authorized the arrest.
Officer Communication and Notes:

Handwritten or digital notes made by the officers, whether in their notebooks or on other devices, pertaining to the arrest and surrounding circumstances.
Any communication between Officer Julia Hall and the Independent Police Conduct Authority (IPCA) related to this case, including phone calls, digital communication, or written correspondence.
Medical and Health Records:

Documentation showing a phone call or visit to a healthcare practitioner in New Zealand regarding my urgent request for blood thinners, which I was denied while being unlawfully detained overnight. I request all related records showing that such a request was made, whether by phone or in writing.
Changes in the Disclosure:

If any alterations were made to the police disclosure after the charge was dropped, I request copies of the updated documents and an explanation for the changes made.
Given the seriousness of these matters, including allegations of police corruption, perjury, and abuse of power, I also request that you treat this as an official request under the Official Information Act 1982.

Please provide these documents in their entirety, including any redactions, and I request that they be made available in an accessible format, either digitally or as hard copies.
