Injury Claim Submission by registered treatment providers who are NOT ACC registered

Dear Accident Compensation Corporation,
How does a registered treatment provider, who is not a registered treatment provider with ACC (not "ACC registered"), file a claim on behalf of an injured person?
Would they still use the ACC-45 form? Or would it be a different form?
What is a non-ACC registered doctor expected to do with a patient, should he discover an injury during regular examination of the patient, if said doctor is not ACC registered?
