Dear Central Otago District Council,

..To whom it may concern,
I am seeking information relating to council regarding My vaccine pass to access council services and facilities. Please return the following information under the Official Information Act.
1. Please supply any long-term safety or efficacy data for any Covid-19 inoculations that the council holds.

2. Who made the decision to implement this policy, and who was involved?

3. Has the council given consideration to the status of inoculations as experimental medical treatments? if this has not been considered by the council, please give details as to why not?

4. Please provide all data, memos, evidence, consultations, meeting minutes and other documents that contributed to the implementation of this policy.

5. Please provide documents for the risk / benefit analysis of the flu, as well as for covid-19.

6. Does the council hold any risk benefit analysis demonstrating that the benefits of this policy outweigh the risks?

7. What is the cost in dollar terms so far for implementation of mandates in the CODC area.?

8. What ethical, legal and human rights considerations has the council given to this policy? if these issues have not been considered by the council, what consideration has the council given to the policies effects, on people to exercise their human rights to informed consent, including rights to informed refusal to medical treatment?

9. As an employer were the rights in point 8 explained fully to the staff? How does this policy respect individual human rights not to be subject to medical experimentation? how does this policy allow people to not be subject to discrimination on the basis of their personal medical status
