    Dear Ministry of Health,
On 7 November I emailed [email address]. In my email I asked about the way vaccinations by DHB are recorded. If I live in the Hutt Valley, for example, but got vaccinated in the Wellington CBD, does that count as a Hutt Valley or Capital and Coast vaccination? I am yet to receive a substantive response.

Thank you for your email on 25 November 2021 where you queried:
 
"If I live in the Hutt Valley, for example, but got vaccinated in the
 Wellington CBD, does that count as a Hutt Valley or Capital and Coast
 vaccination?"
 
Please be advised, the information published on our website is by district
 health board (DHB) of residence unless otherwise
 specified : [1]www.health.govt.nz/our-work/diseases-and-conditions/covid-19-novel-coronavirus/covid-19-data-and-statistics/covid-19-vaccine-data 
 
This DHB of Residence is derived from the address of the person that was
 vaccinated. So, if a person lives in the Hutt Valley but was vaccinated in
 Wellington, this would be counted at Hutt Valley DHB. 
 
The Ministry also captures that DHB of Service. This is derived from the
 location of the site that administered the vaccine dose. This is used when
 reporting how many doses a DHB has administered (e.g. against a plan). 
 
Under section 28(3) of the Act you have the right to ask the Ombudsman to
 review any decisions made under this request. The Ombudsman may be
