Can you please provide the following informaiton on Lumbar Spine Surgery in New Zealand funded by ACC for each of the last 5 years.

1. The number of times the following Core Item ID codes have been approved for surgery per year; SPN240 (SPN115), SPN241, SPN242, SPN243 (SPN116), SPN244 and SPN245, SPN246, SPN247.

Some of these codes may have been preceeded by the code provided in '( )' if these could also be included that would be appericated.
