The Ministry of Health does not hold any information relating to your
request; however, I have been advised that this information is held by the
Waikato District Health Board (WDHB). For this reason, the Ministry of
Health has decided to transfer your request to WDHB under section 14 of
the Act. You can expect a response from WDHB in due course.
