With reference to the following link.

It states:
"The CIR will also record if you decide not to get a COVID-19 vaccine, including if there are medical reasons why you don’t, or if you do not want to be contacted further by the vaccination programme."
Please provide details of;
1) the process of how to provide notice to The Ministry of Health (or other party) to mark persons as "do not contact" under the Covid Immunisation Register.
2) the process to provide notice to The Ministry of Health (or other party) to be deleted from the Covid Immunisation Register as allowed under the Privacy Act.

Thank you for your email on 8 September 2021 and the questions you have
about the COVID Immunisation Register (CIR).
My responses to your questions are as follows:
1. the process of how to provide notice to The Ministry of Health (or
other party) to mark persons as "do not contact" under the Covid
Immunisation Register.
Individuals that do not want to receive any further correspondence via
addressed mail, SMS or email in relation to COVID vaccinations can contact
the COVID-19 Risk and Privacy team at the Ministry of Health. The email
address for the COVID-19 Risk and Privacy team can be found in the
‘Contact Us’ section of this webpage:

We require individuals to provide their full name and National Health
Index (NHI) number, or date of birth (if their NHI is not known). This is
required so their profile can be accurately identified in the CIR.
We will shortly make our ‘opt-out’ process available so that individuals
who do not want to receive further communication regarding COVID
vaccinations can easily advise the Ministry of Health that they want to
stop further communications.
2. the process to provide notice to The Ministry of Health (or other
party) to be deleted from the Covid Immunisation Register as allowed
under the Privacy Act.
The Ministry of Health must hold complete and accurate records of whether
an individual has been vaccinated or not vaccinated. There is no ability
to opt-out or delete your personal information from the CIR as the
Ministry of Health is required to hold this health information for a
minimum period of 10 years under the Health (Retention of Information
Retention) Regulations 1996.

I spoke too soon. The email address given results in an Automatic Do Not Reply message. It points to a "Proof of Vaccination" form. Not what I want.
So it looks like I'll need to get on the phone.

Thanks for making the request Paul. I just sent an email off to get a "do not contact" status against my record. A pity about not being able to get ourselves off it. I might look into the GDPR Right to be Forgotten provisions to see if there is a parallel in NZ Privacy law.
