1. I request a copy of all current contracts, agreements, and similar FENZ has with the Australasian Fire Authorities Council (AFAC).

2. I request a copy of all historical contracts, agreements, and similar FENZ has with AFAC extending back to 1 January 2015.

3. A copy of the SMS reports for incidents with CAD numbers F2716928 and F2866954.

4. A copy of the information supplied to Worksafe for incidents F2716928 and F2866954.
