Dear Accident Compensation Corporation,

Please note the queries below and provide information accordingly

In 2008 the Private Practice belonging to a Hamilton Specialist Psychiatrist was closed . This can be confirmed by the Corporation in a letter dated 18 November 2010 released by the ACC (no reference number visible).

1/ Could you please provide the date Private Practice was closed

2/ Is this still the 'Status Quo' i.e. no changes as far as the ACC is concerned and its dealings with Dr Collier as stated in 18 November 2010

3/ If Practice has reopened since its closure in 2008, when did it reopen and is the Psychiatrist in question currently working in excess of 20 hours per week to the best of the ACC's knowledge

4/ was the Private Practice and/ or qualifications held by the provider pre 2008 applicable to same or similar requirements set out in 4. of 'Service Schedule - Clinical Psychiatric Services 2008'

5/ if practice has remained closed since 2008, has the Psychiatrist been subject to the 'Clinical Psychiatric Services Serviced Schedule' 2008 2009 2010 2011 2012 2013
