Julie,
In response to your email dated 5^th December 2013 requesting information
regarding hip revision surgeries please find our replies to your questions
as below:
Please note that the numbers provided for revision surgery reflect
revisions for all causes; including replacement of joints that had
exceeded the expected life of the prosthesis.
1. Procurement Policy for medical devices
We have eight (8) contracts for hip and knee implants as follows:
Lima Orthopaedics NZ Ltd (signed 29/3/12)
Mathys Ltd (signed 29/3/12)
Orthotech orthopaedics NZ Ltd (signed 29/3/12)
Smith & Nephew Surgical ltd (signed 29/3/12)
Stryker NZ Ltd (29/3/12)
Zimmer NZ Ltd (signed 29/3/12)
Biomet NZ Ltd (signed 29/3/12)
Johnson & Johnson NZ Ltd (signed 1/6/12)
These are national contracts made available by Health Benefits Ltd and
expire on the 30 April 2014 with a right of renewal of two years. The
price schedules were distributed to Tauranga and Whakatane Theatres along
with a “construct table” which groups component prices for various
procedures.
Ownership of consignment products and loan sets remains with the supplier
until, for consignment products, the product is used by the DHB.
2. Funding Policies for medical devices including when manufacturers
will get paid (ie before surgery, upon receiving implant) with specific
reference to custom devices.
We have standard payment terms with suppliers i.e. 20^th month following.
3. How many hip revision surgeries involving an acetabular component
for each year for last 5 years 2012 in region? If not available just
2012.
53 in 2012
4. How many patients received re-revision surgeries in the past five
years? (ie it was their second or third + revision surgery)
We are unable to provide this information. The BOPDHB coding does not
differentiate between a revision and a re-revision.
5. How much was spent on acetabular implants in revision surgeries
over each year for last five years? If not available just 2012.
$169,007 in 2012
6. What is the average costs of running an operating theatre per hour
(minus staffing costs)?
Our costing data includes nursing and anaesthetic technician costs as part
of our theatre cost. Theatre costs approximately $30 per minute including
theatre staff, anaesthetic costs, equipment, recovery, facility costs, and
overheads. The costing excludes anaesthetist and surgeon costs.
7. Total operating theatre costs for revision surgeries in 2012?
Approximately $931,000 including all staffing, implants and facility and
excludes surgeon costs. This amount is for both acute and elective
admission types.
8. How long on average how long does a revision hip
arthroplasty take?
An average of 213 minutes.
9. How much was spent on rehabilitation after hip revision in 2012?
We are unable to provide an accurate answer to this question. We do not
have a separated funded rehabilitation event associate to these
procedures. Rehabilitation forms part of the patients care pathway.
Patients are expected to begin mobilising on day 1 post operatively. This
includes nursing, physiotherapy, occupational therapy assessments and
inputs until discharge. Required rehabilitation varies depending on the
patients associated health factors and age and some patients may require
more mobility training than others.
10. What is the cost of a day of rehabilitation for a hip surgery
We are unable to provide you with this information.

Karina Thomson

Hi Karina Thomson,
Thank you for getting back to me so quickly, that information is great.
Just a quick clarification with regard to total operating theatre costs for revisions, is that for all types of revision surgery such as knees, shoulders, hips etc or just for hip revisions? Sorry I understand my question was very clear so just wanted to check.
Regards,
Julie Hands
