List of statutory Audit obligations of DHB inpatient psychiatric facilities

Dear Ministry of Health,
The purpose of this Official Information Act request is to assist a number of families who have requested my assistance in matters related to several Serious Adverse Events within psychiatric inpatient facilities.
1) Please provide to me (as an electronic document):
i) a list of audits that DHB non-forensic adult inpatient psychiatric facilities have been required to undertake since 2012.
ii) details of whether any of the audits identified in point 1(i) were newly implemented or were revised requirements during the period
iii) details of the frequency in which the audits identified in point 1(i) were required to be undertaken.
2) Please confirm whether the following audits met the minimum frequency requirements for DHB non-forensic adult inpatient psychiatric facilities since 2012 .
- Pharmaceutical fridge - daily temperature log {Done Daily)
- Pharmaceutical fridge maintenance and cleaning log (Done weekly)
- Resuscitation trolley emergency equipment check {Done weekly)
- Defibrillator check (Done weekly)
- Hand hygiene audit-August 2014, May 2016
- Controlled Drug Check - Done weekly
-Controlled Drug Stock Take - Done weekly
- Environmental cleaning and maintenance - service walk through - February 2015
- Health & Safety - planned general inspection by line managers to identify new hazards - December 2013
- MHAID Consumer Information Audit - April 2016, August 2015
- MHAID Inpatient Services Ligature Audit - 2015, 2016
- MHAID Metabolic Monitoring Audit - June 2016
- Medication Administration Audit - January 2013
- Infection Control Audit - July 2014
To better my understanding of the auditing of DHB non-forensic adult inpatient psychiatric facilities, please provide any additional information to that which is requested, including that which may be relevant to the audits that were undertaken in point 2 above.

The information you have requested in part 2 is not held by the Ministry
of Health and would likely be held by the district health boards (DHBs).
Before transferring this part of your request, I wanted to consult with
you on whether you were interested in receiving this information for
certain DHBs, or if you would like this part of your request transferred
to all of the DHBs?

appreciated.
