I request all official information and reports relating to the death of Liam Ashley while in the custody of Dept of Corrections on the 24th August 2006

This includes an assessment carried out by a nurse the North Shore District Court at the request of Liam Ashley's solicitor, also on the 24th August 2006.

Please find attached the response to your Official Information Act request
on Liam Ashley to the Department of Corrections. The Department has
transferred part of your request relating to an assessment of Liam Ashley
carried out by a nurse at the North Shore District Court on 24 August 2006
to Waitemata DHB.
