Dear Earthquake Commission,

I would like copies of all information held by EQC relating EQC claims at 4 Anne Ward Lane, Burwood, Christchurch 8083.

This includes Scope of Works, correspondence, documents and details of work carried out and sign off of works plus cash settlements relating to my property.

Please also provide a breakdown of all payments made to date, for all claims i.e. complete file. Thanks
