Dear Waikato District Health Board,

I respectfully request a response in relation to an admission and subsequent discharge of a patient to Thames Hospital on the 25th of August 2017. The practising consultant was Juan Pablo Meza Budani and the patient was David McVicker

We wish to know exactly what tests were done on Mr McVicker at the time of admission, by whom and what these tests entailed. We wish to know who was also present in the room at the time of the examination and what part they played in his assessment

We wish to ask the reasoning behind why Mr McVicker was discharged at the time of the consultation as there is very limited information contained within the discharge summary - I have the patients authority to access this information however it should have been contained within the documentation that has been forwarded to us and it appears to have either been withheld, not noted, or not carried out
