Reports for blocking of Google IP address.

Dear Department of Internal Affairs,
as per section 5.5.4 of the Digital Child Exploitation Filtering System Code of Practice - March 2013 which states:
"5.4 Inspectors will prepare and maintain a report in respect of each website that is on the filtering list that provides:
the site address by URL;
the date that the site was reviewed and the reviewing inspector;
what an inspector observed on the site each time it is reviewed; and
a recommendation for whether the site stays on the filtering list or is removed."
I would like the report(s) for the website that was blocked on the following IP address: (Which belong to google)
74.125.237.11
74.125.237.138
You should be able to provide this given that the URL has a domain name that resolves to an IP address and the IP address is then added to the filter's BGP feed, causing any traffic to go via FastCom to the filter box where it then checks the http URL.
Undoubtedly you will not be able to share the URL as the filter list is apparently secret, but I would like the rest of the report(s) related to the two IP address above.
The reason for the request is that the DIA is violating section 3.3.3 of the code of practice which states:
"3.3 The DCEFS will not prevent access to any website or impair any Internet traffic that is not clearly within the scope as defined in paragraph 3.1."

and a Telecom technical representive admitting there is a fault, caused by the filter as per the geekzone forum thread,
I am requesting the reports as laid out in section 5.5.4 relating to the above IP addresses and the reasons why the IP addresses were added to the BGP feed, which caused the violation of section 3.3.3.
