Dear Health and Disability Commissioner,
On the 8th of January 2018 I lodged an official complaint to the Health and Disabilities Commission with regard to the Lakes District Health Board and a mutual client
I respectfully request a copy of the information held by the H & D Commissioner in particular the response from the Lakes DHB to our complaint
