1. Medical qualifications held by this Dr.

2. Is the Dr directly employed by ACC.

3. If yes to 2, salary details for each year for the last 5 years.

4. If no to 2, payments made to the Dr as a contractor for each year for
the last 5 years.

5. Copies of all contractual agreements between ACC and the Dr.

6. Period that the Dr has been employed, or if applicable, contracted to
the ACC.
