I would like to make the following information request.

In 2017, DIA produced a report following an inquiry into the Havelock North Drinking Water. Please provide me with all records, including information held in the minds of officials, about the decision to hold an inquiry in this matter. This includes communication among DIA officials, and between DIA and others outside DIA.

I understand that in September 2016 the decision was made to hold an inquiry. My request is concerned with how that decision was made. I require records dated up to and including 30 September 2016. Please note I do note require a copy of the final report.
