Dear Capital and Coast District Health Board,
I would like to request following information under official information act:
1. Count of monthly adverse events from July 2006 to June 2016 for Capital and Coast District Health Board. Please note that prior to 2012 the term used for adverse events was 'serious and sentinel events'.

Also, I do have yearly count of adverse events, but for my research purpose I require monthly events.
