Dear Department of Corrections,
In Inspector Pauline Rimmer's report to the Chief Executive of Corrections, following the death of Tipene Dawson Pomare (aka Te Huna), Ms Rimmer recommended that Corrections establish "whether the practice of full 14 days' segregation for prisoners on initial segregation is widespread nationally and how the management of such prisoners is reviewed/recorded" AND "whether the practice of denying association for prisoners on initial segregation is widespread nationally and how the management of prisoners who are denied the ability to associate with others is reviewed/recorded."
I understand that this review of the use of directed segregation took place.
Please provide me with the report/findings of Corrections' review/investigation of the use of directed segregation following the death of Mr Te Huna.

Thank you for your response.
Please clarify for me the following elements of your response:
1) Did the Department establish whether the practice of full 14 day segregation for prisoners on initial segregation is widespread?
2) Please release the data used to determine this.
3) Did the Department establish whether the practice of denying association for prisoners on initial segregation is widespread?
4) Please release the data used to determine this.
5) Was a policy document created that contained the words "Directed Segregation Review"? Was this document approved by the Chief Executive of Corrections and/or the Minister?
